Indications for the application of oral isotretinoin:

①Severe nodular cystic acne and its variant forms;

②Inflammatory acne with scar formation;

③ Moderate and severe acne that are ineffective to the following treatments: use combination therapy for 3 months, including those who use tetracyclines systemically;

④ Acne patients with severe psychological stress (disfigurement phobia);

⑤ Gram-negative bacilli folliculitis;

⑥ Those who frequently relapse and need repeated and long-term systemic antibiotics;

⑦ Few patients who need rapid recovery for some reason.

Dosage: The usual dose is 0.25～0.5 mg/(kg·d), in order to reduce adverse reactions, the dose should not exceed 0.5 mg/(kg·d). The course of treatment depends on the weight of the patient and the daily dose. The minimum cumulative dose is 60 mg/kg as the target, but if the cumulative dose reaches 60 mg/kg and the satisfactory effect is not achieved, it can be increased to 75 mg/kg. However, even if grade 1 acne is completely cleared, if you stop using isotretinoin before reaching the 60 mg/kg threshold, the probability of permanent cure will be significantly reduced.
