Pharmacological effects:
The active substrate of carbonic anhydrase inhibitors (CAIs) is carbonic anhydrase (CA), which is found in many body tissues, including eye tissues. The main function of CA is to catalyze the chemical reaction of CO2+H2O=H2CO2=H+HCO3. With the increase of intraocular CA activity and excessive HCO3, the osmotic pressure in aqueous humor increases, the generation of aqueous humor increases, and the intraocular pressure increases. There are 7 types of CA, among which the carbonic anhydrase ketone enzyme || (ca-| |) has a higher activity, which exists in the ciliary body and retinal species of human, and is a key enzyme for the formation of aqueous humor. Brinzolamine is a CA-|| with higher selectivity and affinity, which can reduce the secretion of aqueous humor, reduce sodium and water transport by reducing the generation of bicarbonate ions, decrease the osmotic pressure in aqueous humor, decrease the generation of aqueous humor, and finally reduce the intraocular pressure. Brinzolamine can also cause the increase of CO2 in the retina. CO2 ACTS on blood vessels, causing vasodilation and increasing eye perfusion pressure. In vitro, the effective inhibitory concentration of 50% was 3.2nm, and the Ki value of ca-| | was 0.13nm.
Indications:
For the treatment of primary and secondary open-angle glaucoma and high intraocular pressure. It can also be used to prevent intraocular hypertension after laser surgery. It can be used as a single therapeutic agent for patients who are not treated with tv-blockers, or for patients who have contraindications, or as a synergistic therapeutic agent for tv-blockers.
