Meloxicam (meloxicam), chemical name 4-hydroxyl-2-methyl-n -(5-methyl-2-thiazolyl) -2h-1, 2-benzothiazine-3-carboxylamide 1, 1-oxide, is a non-steroidal anti-inflammatory drug. It was first listed in South Africa in 1996 by the German company BoehringerIngelheim. It has analgesic, anti-inflammatory and antipyretic effects, and is suitable for symptomatic treatment of rheumatoid arthritis and painful osteoarthritis. The mechanism of action of nsaids is to inhibit the synthesis of inflammatory prostaglandins (PGs) by inhibiting cyclocytes (COX). Cycdroxygenase (COX) is a membrane-bound serum protein and glycoprotein that catalyzes the oxidation of arachidonic acid (AA) to prostaglandin G2(PGG2), which is then reduced from PGG2 to PGH2. COX has two heterogeneous forms: cyclooxygenase-1 (cox-1) -structural enzyme and cyclooxygenase-2 (cox-2) -pgg2-induced enzyme. The inducible enzyme cox-2 is induced by inflammatory regulation and is involved in the regulation of inflammatory response. The structural enzyme cox-1 is necessary for normal physiological processes and exists in most tissues. Its function is to synthesize PG to regulate the normal physiological activities of cells, protect the gastrointestinal mucosa, support the microcirculation in the kidney and protect other organs from destruction. Among the many different selective cox-inhibitors of nsaids, meloxicam selectively inhibits cox-2, while nsaids such as diclofenac, piroxicam, and indometacin either similarly inhibit two cox-s, or inhibit cox-1 more than cox-2. However, meloxicam is relatively safe in the gastrointestinal tract and less toxic to the kidney, due to its weak inhibition of cox-1.
